Wear The Helmet. The Evidence Isn’t Even Close.
I ride almost every day, and for years I treated the helmet as optional. A short trip to the store, a slow loop around the neighborhood, and the thing stayed on its hook. Then I actually went and read the research, and I have not left the house without one since.
Here is the simple version. If your head hits the ground, a helmet puts a crushable layer between your skull and the pavement. That foam takes time to compress, and stretching the impact over a few extra milliseconds is the whole game in head injury. The mechanism is not mysterious. It is the same reason cars have crumple zones.
The mechanism would not matter if the real-world numbers disagreed. They do not. A Cochrane review of the controlled studies found that helmets cut the risk of head, brain, and severe brain injury by 63 to 88 percent, and that the protection held whether or not a car was involved.
That is one review, and one review can be wrong. So look at a larger and more recent one. A systematic review and meta-analysis in the International Journal of Epidemiology pooled 40 studies covering more than 64,000 injured cyclists. The pattern was the same, and it got stronger as the injuries got worse.

Notice that the worse the outcome, the bigger the gap. That is exactly what you expect from something that works, and the opposite of what you would see from a fluke.
I went looking for the strongest objection too, because a claim that cannot survive its critics is not worth much. The usual one is that helmet studies are biased and that the effect vanishes once you correct for it. The IJE authors tested that directly. They found no real sign of publication bias or time trend, and when they adjusted for it anyway the numbers barely moved. The protective effect did not disappear. It stayed.
None of this says a helmet makes you invincible. It will not save a lower face, it does nothing for the rest of your body, and the smartest move is still to avoid the crash. A helmet is the last line, not the first.
But head injury is where cyclists actually die. Public health agencies say so plainly. The CDC lists properly fitted helmets as a proven way to reduce head and brain injuries, alongside better roads and lower speeds.
So the case is short. The mechanism is sound, the controlled studies agree, the largest pooled analysis agrees, and the effect grows precisely where it matters most. I cannot control whether a driver sees me. I can control what is on my head when they do not. That choice costs me nothing, and the evidence on what it buys is not close.
